Registered Nurse (F/T or P/T)

Brief Job Descriptions:

  • Performs comprehensive assessments of client status, including physical, psychosocial, and environmental parameters.
  • Completes comprehensive assessments including OASIS data set within forty-eight (48) hours of referral unless other arrangements are made.
  • Develops and/or follows an individualized Plan of Care that incorporates analysis of data and current scientific findings.
  • Provides skilled interventions with a focus for achieving realistic client outcomes within a specified time period.
  • Reports changes and information necessary to modify and update the care plan to reflect progress towards goals (outcomes).

Minimum Required Qualifications:

  • Graduate of an accredited school of professional nursing.
  • Current license as a Registered Nurse in California
  • Minimum of one (1) year experience in an acute care setting or equivalent experience, preferred.
  • Licensed driver with automobile insured in accordance with state and/or Agency requirements and in good repair. Good driving record.
  • Current CPR certification.

Licensed Vocational Nurse (F/T or P/T)

Brief Job Descriptions:

  • Provides skilled nursing visits for clients as directed by the RN Case Manager. Follows the care plan established by the Registered Nurse.
  • Recognizes and reports changes in client condition to the Case Manager and physician,
    as directed.
  • Communicates information on a timely basis, consistent with Agency policy and urgency of the situation. Communicates verbally, as needed, and through complete and accurate documentation in the clinical record.

Minimum Required Qualifications:

  • Graduate of an accredited school of professional nursing.
  • Current license as a Licensed Vocational Nurse in California
  • Minimum of one (1) year experience in an acute care setting or equivalent experience, preferred.
  • Licensed driver with automobile insured in accordance with state and/or Agency requirements and in good repair. Good driving record.
  • Current CPR certification.

Physical Therapist (Independent Contractor)

Brief Job Descriptions:

  • Performs physical therapy assessments, diagnostic tests, skilled procedures, and ongoing evaluation for clients who are receiving services under a medically approved Plan of Care.
  • Completes initial assessments within seventy two (72) hours of referral unless other arrangements are made.
  • Completes comprehensive assessments including OASIS data set within forty-eight (48) hours of referral for therapy services unless other arrangements are made.
  • Develops a therapy care plan to provide direction to other health team members involved in the plan of care. Reviews and updates per Agency policy and client need.
  • Documents all findings, plans, interventions, and outcomes and submits documentation to the Agency as per Agency policy.
  • Communicates plans and changes to the physician and to the client Case Manager and other caregivers through the care plan, progress notes, and participation in care conferences.

Minimum Required Qualifications:

  • Has a current licensure as a Physical Therapist in California
  • Minimum of two (2) years experience in healthcare field. Home care experience preferred.
  • Licensed driver with automobile insured in accordance with state and/or Agency requirements and in good repair. Good driving record.
  • Current CPR certification.

Occupational Therapist (Independent Contractor)

Brief Job Descriptions:

  • Performs occupational therapy assessments, diagnostic tests, skilled treatments, and ongoing evaluation of clients who are receiving services under a medically approved Plan of Care.
  • Completes assessments in a timely manner and in accordance with Agency policy.
  • Develops a treatment program and establishes goals for improved function.
  • Provides assessment findings and goals to assist in developing the client care plan and obtains specific physician orders as needed.
  • Documents all findings, plans, interventions, and client progress (outcomes) on designated forms and submits per Agency guidelines.
  • Communicates plans and changes to the physician and to nursing Case Manager and other Agency staff through the care plan, written progress notes, and participation in care conferences.
  • Guides and instructs the client in prescribing therapeutic self-care and creative activities that are directed toward improving independence and physical and cognitive functioning.

Minimum Required Qualifications:

  • Graduate of an accredited occupational therapy curriculum, accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association; or eligible for the National Registration Examination of the American Occupational Therapy Association.
  • Has a current licensure as an Occupational Therapist in California
  • Has a minimum of two (2) years experience in a healthcare facility or equivalent experience.
  • Licensed driver with automobile insured in accordance with state and/or Agency requirements and in good repair. Good driving record.
  • Current CPR certification.

Speech Therapist (Independent Contractor)

Brief Job Description:

  • Performs speech therapy assessments, diagnostic tests, skilled treatments and ongoing evaluation for clients who are receiving services under a medically approved Plan of Care.
  • Completes comprehensive assessments including OASIS data set within forty-eight (48) hours of referral for speech therapy services unless other arrangements are made.
  • Develops a speech therapy plan that identifies functional speech defects or swallowing difficulty and is directed toward improving client function.
  • Reviews and updates the care plan based on client need and Agency requirements.
  • Documents all findings, plans, interventions, and client progress (outcomes) in the clinical record, and submits documentation to the Agency per guidelines.
  • Communicates plans and changes to the physician and to the designated nursing staff through the care plan, written progress notes, and participation in care conferences.
  • Reports changes in client condition or service plan to the Agency supervisor the date changes are noted/made.
  • Implements teaching appropriate to the client’s needs and abilities, and evaluates the effectiveness of the plan.
  • Develops care plan guidelines for other members of the team to support the client goals.

Minimum Required Qualifications:

  • Meets the education and experience requirements for Certificate of Clinical Competence (CCC) in speech pathology or audiology granted by the American Speech-Language-Hearing Association, or has completed the educational requirements for certification and is in the process of accumulating the supervised experience required for certification.
  • Is certified to practice Speech-Language Pathology
  • Has a minimum of one (1) year experience in a health care facility or equivalent experience.
  • Licensed driver with automobile insured in accordance with state and/or Agency requirements and in good repair. Good driving record.
  • Current CPR certification
